- Case Mix: Endoscopy (35%), Urology (25%), Ortho/spine/podiatry (25%), gyn/general (10%), eyes/ENT (5%)
- Support Staff/Supervision: Supervision. 4 Anesthesiologist's and 9 CRNA or CAA's on per day
- # of OR's: 6 OR's, 3 endo rooms
- Credentialing Time-Frame: 7 days
- Certifications: ACLS, BLS, DEA and must be BC
- Active IN License: Required
- Flight/Lodging/Rental Car or Mileage: Yes
